  1. NBI Form No. 5 or the Fingerprint Form : Your Philippine Embassy or Consulate can provide this form to you on request. The form can also be obtained from NBI Clearance Building, 3rd Floor, UN Avenue, Manila, at Mailed Clearance Section. Upon completing the form, be sure to have your signature stamped by your nearest police station or embassy. The person who interpreted your fingerprint impression should sign the form as well. In the Embassy, fees for fingerprinting vary based on the country.
  2. Photocopy of any valid ID issued by the host country or passport : Take along valid passports or IDs from your host country even when traveling abroad. Ensure you reproduce either your bio page or your stamp page showing your most recent departure from your passport.
  3. Photo : A white background and a passport size photo are the standard requirements for NBI clearance abroad.

Authorization letter : Include your signature along with the name and address of the authorized representative who will process the NBI clearance.

To renew your NBI clearance abroad, you will need the following Documents:

  1. Old NBI clearance: When securing a clearance, your representative must present an original copy of your old NBI clearance. The NBI form no. 5 does not need to be secured if you have this document. The old NBI clearance and NBI form no. 5 can both be secured if you want to keep things safe.
  2. A photocopy of the host country’s ID or passport: In the same way you need to provide a photocopy of your host country’s ID to renew your NBI clearance abroad. If you are photocopying a passport, please make a copy of the bio page or the stamp page that shows your departure date.
  3. Photo: For NBI clearance abroad, the photo needs to be 2×2 passport size on a white background, as it is for other legal documents.
  4. Authorization letter : You must affix your signature at the bottom of the authorization letter and specify your representative’s name.
